This consultation has been extended until Tuesday, 8 November 2022.What is this about?
The State Insurance Regulatory Authority (SIRA) has developed a revised allied health treatment request form, formerly known as the allied health recovery request (AHRR). SIRA has engaged with a range of stakeholders regarding issues and improvements to the form.
A summary of the changes being implemented are:
- the purpose of the form is a treatment request only
- integrate the five principles of the Clinical Framework
- include objective measures, standardised outcome measures and risk screening
- reduce the length from 5 to 2 pages
- significantly reduce the capacity section
- simplify the recovery plan.
The allied health treatment request form explanatory notes were developed as a companion document to the allied health treatment request form. These notes have been developed to provide context, guidance and reference material for practitioners to complete the form and are also being updated.
